Pitkin County Copy Request eForm

Please use this form if you are planning to pay for your copy request using a credit card.  We do accept Visa, MasterCard or American Express.  Please note that is an additional 2.7% "portal fee" (see below) added to all credit card transactions.

For security reasons our processing vendor does not allow us to collect your card number online. Staff will process your request and call you to verify the total amount due and to get your card number.  Please DO NOT include your credit card # anywhere in this form!

Standard Real Estate Documents $0.25 / per page
Plats & Survey's $5.50 / per page
Marriage License $2.00
Certification (all documents) $1.00 / per document

Credit Card Portal Fee  - For transactions paid via credit card there is a portal administration fee of $0.75 plus 2.25% of the total transaction amount.  Fee Calculator: The formula used to calculate the portal administrative fees is: .75 + [(Item fees + .75)] x 2.25 

UPS / FedEx - Staff will call you to verify your shipping account number or ask if you want it charged to your card.
